• Brochure or website detailing services and programs provided
• Audited financial statements
• Completed ACORD applications for all exposures
• Program requirements for price, product and service
• Completed supplemental questionnaires (provided by Kraft Lake)
• Brief description of agent’s relationship with this potential insured
